Automatic Payments Terms and Conditions

  1. By enrolling in this recurring payment program, you authorize Evoke Resolutions: (a) to initiate recurring automated clearing house (ACH) debit entries or debit card payments from the checking or savings account you specify, or (b) to initiate recurring charges from your specified credit card.
     
  2. The amount debited from your checking or savings account or charged to your credit card every month will be the monthly charge specified on the package you have chosen to enroll in. Once your enrollment is processed, all payments will be automatically withdrawn from your specified checking or savings account or charged to the designated credit or debit card on a monthly basis following the initial package duration, unless you terminate your authorization in the manner described herein.
     
  3. You agree to be bound by any rules your financial institution requires for pre-authorized electronic funds transfers and/or that your debit or credit card issuer requires for pre-authorized debit or credit card transactions. You are responsible for all fees charged by your financial institution associated with the pre-authorized payment option.
     
  4. You have the right to terminate your authorization for recurring monthly payments at any time following the initial package duration, by logging into your account and selecting the “cancel membership” option.  
     
  5. You must update all changes to your checking/savings account or credit/debit card information by logging into your account.  If you do not update your checking/savings account or credit/debit card information and Evoke is unable to charge your credit card or withdraw funds from your debit card, checking account, or savings account for the amount due, your membership will be terminated. 
     
  6. Evoke Resolutions shall bear no liability or responsibility for any losses of any kind that you may incur as a result of a payment made on items incorrectly billed or for any delay in the actual date on which your account is debited or your credit card is charged. 
     
  7. Evoke Resolutions reserves the right to change these terms or terminate this Program at any time. Notice may be given on your account, via email, or by other methods.

  • The Evoke Resolutions Programs are developed by Board Certified Behavior Analysts (BCBAs). However, Evoke Resolutions isn't a medical organization and does not give medical advice. We strongly urge you to consult with your physician (or primary health-care provider) before starting any new health plan. We also urge you to get periodic medical checkups. These types of changes to body composition can create physical changes that should be medically monitored. Medical monitoring is especially important for people with a known medical condition. The Programs are healthful, but aren’t intended as a medical treatment for any illness or disease. 
     
  • Evoke Resolutions prohibits participation in its programs for individuals under the age of 18, those individuals with an active medical diagnosis of bulimia nervosa, or anorexia nervosa. 
     
  • If you are being treated for an illness, taking prescription medication, or following a therapeutic diet to treat a disease, it's especially important to consult with your doctor and show the Evoke Resolutions Program Goals. Any modifications made to your goals by your physician should be followed.
